God answered my prayers. With his help, I lost 115 pounds. Through his grace, I've kept the weight off since 2002! For nearly twenty-five years, yo-yo dieting was a way of life. I would lose 20 pounds, and regain 30 or 40. Then repeat the same self-destructive cycle over and over again.

At one point, I weighed almost 300 pounds. I suffered with asthma, hypertension, and chronic pain in my back and knees. My clothes got tighter and tighter. I hated to look in the mirror. Every day, little indignities chipped away at my self-esteem:

  • Going higher and higher in the plus sizes
  • Huffing and puffing while walking up a few stairs
  • Dirty looks as I took up more than one seat on the bus or train

At this point, I had to take a look at my whole life – my spiritual walk, emotional state, lifestyle, and physical health. I made a commitment to strengthen my spiritual walk with God which helped me get a handle on my emotional eating. I did tons of research and developed a nutritionally sound food plan that helped me lose weight safely, steadily and without deprivation. By focusing not just on what I was eating, but why, and not relying on my own willpower, but God’s, I was able to lose 115 pounds. I have been at a healthy weight for almost 11 years. I enjoy optimal health. I feel renewed, spiritually led and fulfilled. I am incredibly blessed.

Esther Tambe MS, RDN, CDN, CDE

Esther Tambe is a Registered Dietitian and Certified Diabetes Educator. She received her Masters of Science in Nutrition with a concentration in Exercise Science at Long Island University. She also received her Bachelors of Science in Nutrition from Long Island University and Bachelors of Science in Health Science-Public Health at Stony Brook University. Esther completed her dietetic internship at Long Island University. When Esther is not talking nutrition & optimal health, she is traveling to new countries on her bucket list.

Debrah Sarria a bilingual (English & Spanish) Nutritionist, Specializing in Integrative Nutrition, Weight loss, Gastrointestinal issues & Food intolerance. My story began a few years ago when my health deteriorated, I was overweight and tired of visiting different doctors that could not diagnose what was wrong with me. Apparently, I was suffering from Hypothyroidism, but the treatment for this condition (daily dosage of Synthroid) did not work. That is when I decided to investigate, study, and learn everything related to autoimmune diseases, integrative nutrition, functional medicine, and the effects that food has on our bodies.

After several years of studying, I managed to diagnose and treat my health problems through a personalized nutritional plan. I was able to overcome digestive disorders such as leaky gut, insulin resistance, fatty liver, and hypothyroidism. I learned how certain foods affect the body negatively and concluded that in order to be healthy, each person needs an individualized nutritional plan. I do not believe in diets or willpower, just proper planning. I believe it is possible for anyone to lose weight with the proper plan, and dedicated support, and that is my mission here at Elizabeth Madison Nutrition. As a part of team Madison, I am here to help you succeed on your wellness journey.


Patricia Cooke MS, CDN, CNS, CDE, CBC

I am a NYS Licensed Certified Dietitian Nutritionist. I am certified in Adult Weight Management by the Commission on Dietetic Registration. I am also a Certified Diabetes Educator and a Certified Bariatric Counselor. I received my bachelor’s degree in nutrition from Queens College and my master’s degree in Clinical Nutrition from New York Institute of Technology. I have worked for over thirteen years counseling patients for a variety of health issues and chronic diseases with a specialty in clinical weight loss, weight management and bariatric surgery. I have worked extensively with all ages in achieving the healthiest body they can through lifestyle and dietary changes while adopting healthy attitudes towards weight and food.

My goal as a nutritionist is to help people have the healthiest body they can have with the resources available to them. Being healthy and eating good food should not feel like a chore. Foods that we eat can play a vital role to alleviate symptoms brought on by a variety of health issues and in some cases help treat conditions themselves. Never underestimate the power of good food!

Angelique Martin, Registered Dietitian

Angelique Martin, RDN, CDN

I am a Registered Dietitian and New York State Certified Nutritionist.  I graduated from Brooklyn College – CUNY with a BSc in Food and Nutrition.  I am certified by the Commission of Dietetics Registration in Adult Weight Management.  

I enjoy working one-on-one with clients to create personalized and realistic eating plans that will meet their needs, lifestyle, cultural and personal preferences.

It is my belief that good nutrition and proper diet are essential for improving our health and quality of life. Healthy eating habits and better food choices play essential roles in building and/or maintaining the foundation of good health. 

I am a member of the Caribbean Association of Nutrition and Dietetics, the Academy of Nutrition and Dietetics and the Greater New York Dietetics Association.


Elise Harrisom

Elise Harrison, Social Media/Marketing Assistant

I am an aspiring registered dietitian nutritionist currently pursuing her master’s in health and nutrition sciences. I am currently a dietetic intern at Brooklyn College, co-chair of the student committee and a member of the social media committee and public policy team at the Greater New York Dietetic Association (GNYDA). I love sharing science-based information via social media. It allows me to reach a larger audience and hopefully move individuals to the contemplation phase regarding their lifestyle and diet decisions. I host local meetups for the National Organization of Blacks in Nutrition and Dietetics (NOBIDAN) and for Diversify Dietetics, a nonprofit organization that is dedicated to increasing diversity in the field. I have a strong research background, a passion for integrative and functional nutrition/medicine, as well as an interest in community nutrition, public policy and bariatric surgery.

Shanita Smith, Office Manager

As the Office Manager for The Weight Loss Transformation Center, I oversee office
operations and ensure that everything runs smoothly. I am committed to making sure
our clients and their families’ have a positive and welcoming experience in our office. It’s
important to me that each of you feel respected, comfortable and relaxed while you’re
here. I joined The Weight Loss Transformation Center/ Elizabeth Madison Nutrition
PLLC. in 2016 and have over 10 years of office operations experience and customer
service. My goal is to incorporate the wealth of knowledge and customer service that I
have acquired to equip our team to best serve our clients and their families.

I’ve spent the past decade working in the wellness industry in an administrative and
support role. I’m passionate about wellness and helping people have the health and life
that they desire. My passion for wellness and helping inspired me to become a Certified
Health Coach. I work with individuals to guide them towards creating the lifestyle that
they desire through habit change. In the Weight Loss Transformation Center, I provide
health coaching to the WLT Warriors to support their faith-based wellness journey.
